The Purpose of This Guide
Some transmogs are only available from a specific non-repeatable quest in the game. Later you might later realise that the old junk you vendored is something you want for transmog, but it cannot be obtained again.
Additionally, Mists introduced a 'smart' quest-reward system which will only reward you items that you can use in your current spec. The downside is that many people inadvertently missed out on transmogs because they were not shown in the list of quest rewards for their current spec.
This guide is intended to be a handy reference for anyone who does not wish to make the same mistakes as they level their characters through Draenor.
I am only listing items that cannot be farmed anywhere else. All items not listed have an identical BoE or dungeon drop counterpart.
N.B. This guide is correct as of WoD launch. Also note that some gear screenshots on Wowhead were from early in beta, and are now outdated!
UPDATE FOR LEGION:
With the new transmog system, missing unique models is no longer a concern! Models from completed quests will automatically be added to the transmog library. Be sure to complete all the quests with unique rewards in time for the xpac :)

) is one of the first quests you'll aim to do when you hit level 100. It rewards some exclusive weapons. However, not all of the rewards are unique, so be prepared to respec and choose carefully.

, but since it's from the Challenge Mode daily bag, it's hit-and-miss.

It's identical to
Hoof of Yalnu
, a dungeon drop, but the smart loot system makes it only available for Intellect-users, making it a rogue's best chance for this model. However if you enter the dungeon normally, without using the dungeon finder or with a full party, you can farm it by switching the 'personal' loot system off.
Note:
Skullcutter of Trials
is identical to
Pillar of Malice
(a BoE drop available to anyone), so don't pick this!

Note that several of these cloaks come from the

quest, meaning you have to make a hard choice between them!
